Well... one of the things I like most is just the fact that he indeed has not only a "burst of joy" at the end of the story, but that in fact "everything can be possible" for him. He has made his decision to leave already, and staying in the Shire and seeking for a fulfilling life at home is no longer an option. He will go, and he knows it.
But in this blessed moment he feels that still "everything" may have a good ending... his Sam finds a rich and happy life with the woman he loves, the Shire will prosper again and yes... he will be healed, too, not in the Shire, but in the Undying Lands. I remember a discussion with illyria_novia about the topic if Frodo was ever happy again. At the end of this fic I would say. Yes. Yes he is happy, and he finally sees the right way for him and a clear amd shining path into a future that includes himself being free, complete and at peace.
